Pay Nomination Fee Online - $100
Hudson Valley Magazine’s Excellence in Nursing Awards Advisory Committee reserves the right to decline a nomination if the nurse does not meet the basic criteria. Nomination must be made by a nursing employer (patient/family nominations must be submitted through nurse’s employer).
The first 100 nominations received by February 15, 2010 will be accepted. $100 nomination fee (includes dinner at the May ceremony and all recognition materials).
If unable to accept submission, the nomination fee will be refunded. Digital headshot photographs (emailed or sent on CDs must be received with nomination to be included in presentation. Photography may be emailed as JPEG file (3”x 5”, 300 dpi). Email all headshots to pr@hvmag.com.
Nurses will be notified, by mail, of award and event details.
