We want to celebrate those who consistently put others before themselves through hard work and dedication to their job of being a nurse. Hudson Valley Magazine is proud to bring you the 4th Annual Excellence in Nursing Awards, so that we can all take the time to recognize the lengths that these individuals go to in order to provide the best care for their patients.
The recipient of an Excellence in Nursing Award is someone who might be called a guardian angel, who has pursued higher education and worked with the families of those who are ill in order to improve the lives of others. We here at Hudson Valley Magazine would like to honor those nurses at our Excellence in Nursing Awards Ceremony whereby the magazine will pay tribute to the winners and give them the recognition that they deserve.
During the 2011 National Nurses Week the magazine will formally honor the award winners at a special celebratory dinner in May. The nominations are open to their supervisors at hospitals and health care facilities, who work with the nurses who go above and beyond to make a difference. The Top 20 nurses chosen will be profiled in the May issue of Hudson Valley Magazine.

Celebrate 2011 National Nursing Week
Nurses receiving Hudson Valley Magazine’s Excellence in Nursing Awards will be individually recognized at an awards dinner and invited to receive a certificate of recognition. This award is open and exclusive to all healthcare settings - hospitals, home healthcare, health centers, schools, health departments, long-term care, nursing schools, agencies, and medical practices. All are warmly invited to participate.
NOMINATE A NURSE FOR RECOGNITION
Criteria for nomination:
• RNs and LPNs (full or part-time)
• Made a significant impact on patient care
• Gone “beyond the call”
• Demonstrated excellence
• Shown commitment to the community served
• Achieved life-long professionalism
IMPORTANT DETAILS:
• Hudson Valley Magazine’s Excellence in Nursing Awards Advisory Committee reserves the right to decline a nomination if the nurse does not meet the basic criteria.
• Only the nurse’s employer may submit a nomination (patient/family nominations must be submitted through nurse’s employer).
• The first 100 nominations received by February 14, 2011 will be accepted.
• $100 nomination fee (includes dinner at the May ceremony and all recognition materials).
• If unable to accept submission, the nomination fee will be refunded.
• Digital headshot photographs (attached to the form below or sent on CDs) must be received with nomination to be included in presentation. Photography may be attached as JPEG file (3”x 5”, 300 dpi). Attach all headshots below.
• Nurses will be notified, by mail, of award and event details.
